If a patient needs help joining their visit, BridgeCare coordinates directly with your team to align the physician visit with the home health start-of-care or scheduled visit. Simple steps: 1. We confirm timing with your team 2. The visit is aligned with your scheduled patient visit 3. Your staff can assist the patient in joining the video visit 4. The physician completes evaluation and orders during the visit For patients not comfortable with technology, we coordinate directly with your team so the visit happens smoothly. The patient's chart is prepared in advance—diagnoses, medication list, and medical history summary. The physician visit is aligned within your team's visit window. Your staff assists the patient via a pre-sent video link on phone or tablet. The physician completes a focused 10–15 minute evaluation and provides start-of-care orders during the visit.
