Meet the BridgeCare Team

Dr. Harshini "Hershey" Jayasuriya, MD, FAAFP spent over a decade working across inpatient, outpatient, PACE, and medical management before founding BridgeCare Medical in 2020. Throughout that time, she saw the same system failure repeat itself: patients were discharged from hospitals and emergency rooms without a physician in place to write the home health orders required for care to begin. Home health is designed for patients who cannot easily leave their homes—yet they are expected to navigate a fragmented system just to access it. Families were left without answers, agencies were ready but waiting, and patients often returned to the hospital weeks later with conditions that should have been managed at home. Recognizing this as both a clinical and operational gap, Dr. Hershey left her role as a hospital Observation Director and self-funded the launch of BridgeCare at a time when telehealth was just beginning to scale. Her goal was simple: ensure that no patient is discharged without the physician oversight required to start and sustain home-based care. In its first year, BridgeCare supported over 1,000 patients and achieved a 2% 30-day readmission rate—significantly below national averages of >15%—demonstrating the impact of timely, physician-led coordination. Today, Dr. Hershey continues her work as a home visiting physician and leader with Harmony Cares while overseeing BridgeCare's growth as a scalable solution for health systems, home health agencies, and payers seeking to reduce readmissions, improve discharge efficiency, and control post-acute costs. BridgeCare is now a network of providers and coordinators who initiate and manage the orders that make home health possible—fast, covered, and physician-supervised from the first order through the course of care. Dr. Hershey lives in Greater Lansing with her husband, two daughters, and menagerie of pets.
